Development of the Residual Lesion Score for congenital heart surgery: the RAND Delphi methodology

Background and Objective:
The Residual Lesion Score is a novel tool for assessing the achievement of surgical objectives in congenital heart surgery based on widely available clinical and echocardiographic characteristics. This article describes the methodology used to develop the Residual Lesion Score from the previously developed Technical Performance Score for five common congenital cardiac procedures using the RAND Delphi methodology.
Methods:A panel of 11 experts from the field of paediatric and congenital cardiology and cardiac surgery, 2 co-chairs, and a consultant were assembled to review and comment on validity and feasibility of measuring the sub-components of intraoperative and discharge Residual Lesion Score for five congenital cardiac procedures. In the first email round, the panel reviewed and commented on the Residual Lesion Score and provided validity and feasibility scores for sub-components of each of the five procedures. In the second in-person round, email comments and scores were reviewed and the Residual Lesion Score revised. The modified Residual Lesion Score was scored independently by each panellist for validity and feasibility and used to develop the “final” Residual Lesion Score.
Results:The Residual Lesion Score sub-components with a median validity score of ≥7 and median feasibility score of ≥4 that were scored without disagreement and with low absolute deviation from the median were included in the “final” Residual Lesion Score.
Conclusion:Using the RAND Delphi methodology, we were able to develop Residual Lesion Score modules for five important congenital cardiac procedures for the Pediatric Heart Network’s Residual Lesion Score study.

Leveraging elements of game design and theories of human motivation, gamification provides a variety of techniques to engage learners in novel ways. Our Clinical and Translational Science Award created the software platform (Kaizen-Education©) to deliver gamified educational content in 2012. Here, we explore two novel use cases of this platform to provide practical insights for leveraging these methods in educational settings: (1) national training in rigor, reproducibility, and transparency and (2) attainment of learner competency (n = 7) as a gauge of curricular effectiveness across Master of Public Health degree tracks (n = 5). Data were captured in real time during player interaction with Kaizen-Education© to provide descriptive analyses of player engagement in both implementation examples. We then assessed item analysis to assess knowledge gain and competency attainment. We have just begun to leverage the potential for gamification to engage learners, enhance knowledge acquisition, and document completion of training, across various learning environments. We encourage a systematic approach to gamification applying insights from self-determination theory to learners and learning environments, a methodical approach to game design and rigorous analysis after implementation to generate evidence-based insights to maximize educational return for time invested.

Critically significant parental effects in behavioral genetics may be partly understood as a consequence of maternal brain structure and function of caregiving systems recently studied in humans as well as rodents. Key parental brain areas regulate emotions, motivation/reward, and decision making, as well as more complex social-cognitive circuits. Additional key environmental factors must include socioeconomic status and paternal brain physiology. These have implications for developmental and evolutionary biology as well as public policy.

Abstract. Poliheuristic theory (PH) is an innovative attempt to conceptualize decision making in a way that recognizes patterns deriving from both cognitive and rational schools of thought about how foreign policy is made. Crisis decision making, a setting in which the political aspects emphasized by PH can be expected to operate, is the subject of this article. The study focuses on Turkey and China, two important states that frequently are characterized as sui generis and possibly unsuited to comparative analysis in the context of any overarching theory. As will become apparent, PH is capable of explaining crisis decision making for China and Turkey and the common pattern we explore is perhaps only the tip of the iceberg when it comes to cross-national application of the theory within the crisis domain.
Résumé. La théorie poliheuristique (PH) est une tentative novatrice de concevoir la prise de décision en reconnaissant l'apport des modes de comportement issus des écoles de pensée cognitive et rationnelle. Cet article porte sur la politique étrangère en temps de crise et offre un cadre politique au sein duquel les modes de comportement identifiés par la théorie poliheuristique devraient normalement opérer. Plus particulièrement, l'étude se concentre sur le cas de la Chine et de la Turquie, deux États souvent considérés comme singuliers et possiblement inadaptés à l'analyse théorique de la politique étrangère comparée. Toutefois, comme cet article le démontre, la PH est en mesure d'expliquer de manière comparée la prise de décision de la Chine et de la Turquie en temps de crise. Les conclusions de recherche laissent penser que cette étude pourrait bien être le prélude de nombreuses recherches employant la PH dans le domaine de la politique étrangère comparée en temps de crise.

The purpose of this article is to present and establish the significance of the Nuclear Arsenal Game (NAG). The NAG investigates behaviour within dyads composed of states that experience an international crisis. It assumes that nuclear and quasi–nuclear states act according to the size and potential of their own nuclear force structure and to that of their opponent. This article argues that the size and potential damage an arsenal poses, set in relation to an enemy state's second–strike capability, determines actor preferences within a crisis situation, preferences which include the option of launching a first strike and risking retaliation in kind. The primary purpose here is to propose a nuclear index for use in empirical studies and offer an example of a game–theoretic model of crisis interaction based on the Theory of Moves (TOM) that considers (a) all levels of nuclear capability; and (b) Classical (or Rational) Deterrence Theory (CDT) to be effective in identifying and explaining actor preferences and predicting behaviour that could include intentional nuclear use.
